Here is my expectation:
Tenma'll go back to japan and start to realize that Harima is the one that always want her to be happy. Probably after she start to fall in love with Harima, something magically cured karasuma, but at that time Tenma is already in love with harima forcing karasuma to give up on her...
Trust me, karasuma's sickness will be cured for sure!
Tenma will stay at Karasuma side..something that he didn't want her do to but he can't take that privilage from her...
^ i agree, just like harima, tenma has an unwavering heart.. she only fixed his eyes on karasuma the whole time and because of that, failed to see that others have fallen for her especially harima.
Basically Karasuma will be alive but he's moving on instinct?

From the clues given, the normal prognosis is that Karasuma has Early-onset Alzheimers, also known as Familial Alzheimer Disease. I won't waste forum space since you guys can just google those names if you're interested. The first time I heard about it was in a Korean movie titled, "A Moment to Remember". Anyways, if that's what he's got, making new memories isn't going to make much of a difference.
That being said, plot and character development aside, I kind of want KJ to warp the situation into a misunderstanding. I've always been an Onigiri fan, but I've also had a soft spot for Tenma (Just not one that let her be with Harima). If Karasuma really has Alzheimer's, then Tenma will probably just stay by his side dutifully. She has a history of caring for objects that are incapable of showing their gratitude (Like the b chapter where she cared for the non-blooming sakura tree), and I would rather not have her spending the rest of Karasuma's life as a human get-well soon present.
such a brilliant point u've got there FallingFurther...Tenma is like that...blindly caring for something...and from the other perspective, i do think karasuma will recover (even though not fully-recover) under tenma's care...just like what happened to the sakura tree...in short, we are hoping for miracle to occur again....hey...it's School Rumble we're talking about here...full of miracles...
