All that matters right now is that certain things are finally coming to a head, and i'm looking forward to seeing a (hopefully) less tenma-struck Harima after this arc.

the ROOT of all problem is Harima didn't confess properly to Tenma. Man,after 262 episode, an unsung lover.sigh

That really isn't the root of all the problems. Harima confessing to Tenma, during any point of this manga would not have turned out favorably for Harima. Also, confessing in this current arc wouldn't have turned out favorably for Harima either, and it would have gone against his character of wanting Tenma to be happy. He chose to back down on his latest confession attempt because it wouldn't have been best for Tenma to hear it. Right now, and throughout the series, Tenma has been obsessed with Karasuma. Although she does socialize with Harima, she has never looked at ANY man in that way. A confession from Harima would have just brought about the same response that she gave to that big dude early in the series. Besides, she still thinks that Yakumo and Harima like each other (half right, on Yakumo's end). So, a confession from Harima would probably make things worse at this point.

If there is anything that could be blamed for all of this, it could be Harima and Tenma's very first encounter (the one when Harima saved Tenma from a thug, only to later be confused as a pervert). If Tenma actually took the time to listen to his explanation at that time, or if she is to actually remembers that it was Harima who saved her, then maybe things could have turned out differently. It's impossible for the first to happen, but there is still time for Tenma to remember what Harima did for her back then (and maybe that'll open a whole new set of realizations). I don't know, I still think there is sometime left before this series ends. Too many questions left unanswered, and not only dealing with this situation.

It's in the unnumbered chapter (also referred to as ch. 16.5). It's the chapter where Harima and Imouto are sitting in a restaurant and he tells her about the first time he met Tenma, but not realizing that Tenma is sitting right behind him. Thankfully (or maybe not) she is too dumb (and drunk at that point) to put two and two together. Instead she said, "Who is this person~? That girl was awful!"

In manga that chapter is from Volume 02 Chapter 00: The Time Machine. You can find it in this thread: Missing Chapters/Newest Releases <-----DOWNLOAD HERE

In anime it is in the first part story of Episode 5 Season 1.
