Yeah, it's been set up since around chapter 30 when Tenma and Karasuma had the infamous split over curry and Harima wanted to fight him.

Is it ok for Harima to fight a terminally ill person?

it kinda looks to me as if the story goes from right to left... first starting with Harima finding out about Tenma's grief... the punch to Karasuma and Karasuma's response to the head after a second failed attempt by Harima trying to punch him...

After all that Karasuma tells Harima the truth... Harima's face with shock at the end.

That looks more like bloodlust than shock at the end to me. Harima shouldn't be smiling like that if he just found out the truth.
