Option 1: He uses genjutsu and wasn't really hit.
Option 2: He uses one of Orochimaru's replacement techniques (spit himself out of his body?) just in time.
Option 3: His mangekyou has a technique which can heal his wounds.

Whatever the option, Sasuke is 100% okay.

Option 2 is no longer possible since Oro is dead; Option 3 would be a little gay, a waste of an MS tbh; Option 1 is hard to pull on a blind person who you've yet to have physical contact with.
