Totally evil and insane.

Kabuto used to be my favorate character until I saw this. Kishi just ruined him entirely by doing this to him. He was sopposed to be the shady right hand man who seemed to have his own ideals in mind but now...ugh. There are no likable characters in this manga now.

I dont understand that language but it looks to me that Kabuto doesn't look crazy or evil but actually has Orichimaru for the left side of his face.

Well, that's what the spoilers seem to say, at least.
And that grin does make him look evil and crazy to me. XD
