Start from the beginning of the manga because the anime did change things - the only manga that I'd say you could skip parts of the manga after watching are Claymore and Death Note (of the ones I've seen). Sure, you may go over things that you saw in the anime, but I personally think it's more enjoyable, particularly for a manga like Ouran that, as Shadowbox said, doesn't really have a plot.
Also, there are filler-ish episodes in the anime; the Alice in Wonderland storyline did not resemble the original in any way except for the basic idea of an Alice in Wonderland story. I guess it could be said that the manga has filler too, but it isn't quite true that the anime follows the manga exactly.
