Tch fook, it's translated already... well at least the old version that does not include the step-by-step on chapter 270 is translated. Scroll down the page and you'll find "english version" somewhere on the left hand side.
As for where this comes from, it's part of the "bonus pages" you get when you buy the "volume" edition of gantz.

Well, actually, all of it is not part of the bonus pages per se. I'm fairly confident Kei-Kun made that Izumi aiming at random guy pic himself. The explanation, however, builds on the contents of the bonus pages, especially those that actually deal with drawing methods e.g. Volume 1, as opposed to those that are only fanservice e.g. 16 (MOAR REIKA). As for the Step-by-step, I am guessing it comes from either volume 24, or from a young jump article (somewhat unlikely - Kei-Kun would most probably have mentioned it)
Ok actual picture 9 should be volume 24 bonus. As for when it happens in the manga, it should be the Nuri fight (I'm guessing where Nuri turns into that "thing", just after oka lands on the bridge (or just before). It's probably a small box in a bigger page no one ever paid much attention to until now.
Edit: Actually, it's a massive one page. Chapter 270, page 18 (or 17, or 19, depending on your sub). Seriously people, try harder. Don't just go "i don't remember seeing this before, so it can't be anywhere in the manga".
It would be interesting to know what kind of program Oku is using. He puts a lot of effort in a manga that's serialized in a magazine every second week.
