Hospitals don't clean needles at all, they use new ones every time.Only the U.S., Canada and certain European countries actually follow proper disinfectant procedures. I know an entire generation of AIDS victims in the U.S. were given it by blood transfusions - back before the U.S./other countries started screening blood donors.
Then of course theres children who got HIV/Aids for the mother (which is pretty much guaranteed if infected before/during the pregnancy)...
And of course theres just the plain unlucky people who get splashed with blood/have blood introduced other ways in everyday fights, accidents, being raped etc etc, things they don't always/usually have a choice over...

P.S. The illegal drug part is totally accurate though. You mess with shit that can kill you (Overdosing etc) don't be surprised if it kills you a different way (AIDS).
