Well, You can't establish some sort of ordering of who is the most powerful in this manga. Add to that the fact that Kubo is using a very used up style of writing in this manga which makes it incredibly easy to predict who is going to win in a bout.
Well, the manga is inconsistent, ill give you that. Some of the things you mention can be explained though, at least where Ichigo is concerned. When he fought Noitora (the four armed thing), he had just finished fighting Grimmjow and was clearly spent from that fight, so he had almost no chance against a more powerful opponent. Technically, the fight Ichigo had with Kenpachi ended in a draw, and don't forget that Kenpachi didnt use that 2 hand kendo grip against Ichigo that he used against Noitora. Also, Ulquiorra has mentioned that Ichigo's power fluctuates wildly between weaker than his own and stronger than his own, so that can pretty much explain any inconsistency in Ichigo's power. Other than that, I can only agree on the inconsistencies.
It's the wonderful skill of writing. The bait and switch will last forever. Keeping the writer's pocket warm with cash. =/

Well Rukia getting beaten by that hollow in the beginning was just a plot device to get the story running and to give Ichigo shinigami powers. Not that this or the fact that she had to step in to save Ichigo makes it more plausible. We all know Kubo is no genius when it comes to writing a stroy. He can draw interesting fights, so Bleach has 95% fights and 5% story/character development.
The sooner you accept that the sooner you'll stop thinking about these fluctuating powerlevels.

Eh.. its ok. Although I just noticed that the drawings were pretty minimalistic compared to other mangas out there. Maybe he has less assistants..
Well, he's not good at writing stories, imo, but I think he's far better at writing poems
